JOB SUMMARY

Psychosocial Rehabilitation Services specialist at our community behavioral health services
agency is dedicated to helping individuals with mental health challenges develop essential life
skills, regain independence, and integrate into their communities. This role involves providing
clients support, guidance, and empowerment as they work towards their recovery goals.

RESPONSABILITIES
  • Conduct comprehensive assessments of clients; strengths, challenges, and goals related
    to their mental health recovery.

  • Develop individualized psychiatric rehabilitation plans that outline clients; goals, objectives, and strategies for skill development.

  •  Assist clients in building and enhancing life skills, including communication, problem-
    solving, coping, time management, and socialization.

  • Facilitate group sessions to promote skill-building, mutual support, and community
    integration among clients.

  • Collaborate with case managers, therapists, medical professionals, and other team
    members to ensure coordinated and holistic care.

  • Provide psychoeducation to clients and their families about mental health conditions,
    coping strategies, and wellness practices.

  •  Offer crisis intervention and immediate support to clients in emotional distress or crisis
    situations.

  • Monitor clients; progress and adjust PRS as needed based on their evolving needs and
    achievements.

  • Empower clients to set and achieve their recovery goals, promoting a sense of
    autonomy and self-advocacy.

  • Maintain accurate and confidential client records, documenting progress, interventions,
    and outcomes.

  • Participate in interdisciplinary team meetings and case conferences to share insights
    and contribute to clients; care plans.

QUALIFICATIONS
  • Bachelor's degree in psychology, social work, rehabilitation counseling, or related field.

  •  Experience in psychiatric rehabilitation, mental health support, or related roles.

  • Strong understanding of recovery-oriented principles and practices in mental health
    care.

  •  Excellent interpersonal skills to establish client rapport, promote empowerment, and
    facilitate group interactions.

  • Compassion, empathy, and a commitment to fostering clients; independence and well-being.

  • Ability to work collaboratively within a multidisciplinary team and communicate effectively with diverse individuals.

  •  Familiarity with mental health resources and community services to connect clients
    with relevant support systems.

  •  Proficiency in maintaining accurate documentation in electronic health records systems.

WORKING CONDITIONS

The psychiatric rehabilitation specialist will work within a community behavioral health services facility, engaging with clients both individually and in group settings. Flexibility in scheduling may be required to accommodate clients; availability, including evenings and weekends.
