Targeted Case Manager

The Targeted Case Manager helps clients navigate behavioral health care and access the services and resources that support recovery and stability. This role builds strong working relationships with clients, families, providers, and community partners.

KEY RESPONSIBILITIES

• Assess client needs, strengths, challenges, and goals.

• Develop person-centered care plans with clients and update them as needs change.

• Coordinate services among behavioral health providers, medical providers, and community resources.

• Guide clients and families in navigating systems of care and available supports.

• Advocate for timely, appropriate services and assist with access to housing, employment, financial, social-service, and other community resources.

• Provide support during crises and monitor progress toward established goals.

• Maintain accurate records of contacts, plans, referrals, and progress.

• Participate in team meetings and case conferences to support coordinated care.

QUALIFICATIONS

• Bachelor's or master's degree in social work, psychology, counseling, or a related field.

• Required state credential, certification, or training, when applicable.

• Experience in case management, care coordination, or behavioral health services preferred.

• Knowledge of community resources, strong organization, and effective communication skills.

• Ability to advocate respectfully, manage multiple cases, and document services accurately in electronic health records.

WORK ENVIRONMENT

This role includes in-person and remote contact with clients and may require travel to client visits, community locations, or meetings. Flexible scheduling may be needed to support client access to services.
